Congratulations on your SIS scholarship! Please complete the form below to ensure that your scholarship is properly processed. The deadline for completing the acceptance form is Wednesday, June 10, 2020.

The award will be applied to the term of your choice for the upcoming academic year: Fall 2020 or Winter 2021.

Please note that SIS scholarships are tuition-based awards. If you receive other tuition-paying benefits, you will be unable to retain your SIS scholarship as these awards only pay for up to 100% of tuition charges. No refunds on excess award amounts will be issued. For example, this would include Graduate-Professional Scholarship recipients as well as Graduate Student Assistants (GSAs).
